Application forms
Complete the following three forms, including one online form and three downloadable PDFs.
- FAFSA on the Web (online form) - All students must complete this form for each year they apply for financial aid. Use the school code 001739.
- Verification of Employer Educational Assistance (pdf)
2011-12 Academic Year
- University Aid Application (pdf) This form is available as an online application for students who have their NetID and password.
- Professional Development/Non-degree University Aid Application (PDF)
Additional forms
These forms do not need to be completed unless the financial aid office specifically requests them. Make sure you complete it for the appropriate year. Fill it out for the year to which you will be applying the financial aid, which might not necessarily be the current year.
