Forms

The Chicago Office of Financial Aid cannot accept any form that contains an electronic signature (unless you are completing the online University Aid Application via your To-Do list in CAESAR). Please print out any required form, complete and sign the form, then return it to our office via fax or email.

If you are submitting a form via email, attach the form to the email as a pdf. We are unable to accept any forms that are submitted as a picture or image file.

These forms do not need to be completed unless the financial aid office specifically requests them either through your To-Do list in CAESAR or via email. 

Application forms

See application instructions for steps and required forms. All federal aid applications begin with your FAFSA. Required forms will also appear on your To-Do list in CAESAR. If you do not see these forms in CAESAR, contact our office.

Federal Verification forms

Verification is the process by which Northwestern University confirms accuracy of the data reported on an individual student's FAFSA. If you've been selected for this process, view the Federal Verification page for required forms and information on what to expect.
